HIV Housing Programs - AIDS Foundation of Chicago - Main Location

AIDS Foundation of Chicago's supportive housing programs are collaborations of community-based social service providers that simultaneously care for the medical and housing needs of people who are chronically ill (with HIV) and homeless. The project partners believe that when homeless individuals with chronic medical conditions are able to focus their energies on healing and maintaining their health, rather than coping with the challenging circumstances that arise from living on the streets and in shelters, their health will improve.

Permanent Supportive & Affordable Housing - Lincoln Park Community Services - Old Town

LPCS facilitates independent housing and support services for 17 individuals living with disabilities through the Independent Community Living Program (ICLP). Additionally, LPCS houses and supports 20 individuals through project-based units at their Sedgwick Old Town location. They serve 37 individuals annually through permanent supportive and affordable housing, several of whom are graduates of the interim housing community.

Housing Services - Impact Behavioral Health Partners - Main Location

Impact Behavioral Health Partners provides stable, permanent, supportive housing and incorporates clinical and supported employment services to provide a well-rounded, holistic approach to the recovery of individuals with mental illness. Utilizing the "Housing First" methodology, their programs provide affordable housing to low-income individuals, many of whom are chronically homeless, and have chronic mental illnesses. They offer a stable foundation so residents can focus their energy on recovery, rather than where they will find shelter for the night.

Provides housing for chronically homeless adults. This program allows for a focus on specific Chronically Homeless sub-populations in need in the community, such as women, persons with mental illness, substance abuse problems, or dual diagnosis; veterans; or those with HIV/AIDS.

EMERGENCY FOOD AND SHELTER HOMELESS PREVENTION SERVICES | MCS COMMUNITY SERVICES

Provides low-income clients with rent and security deposit assistance to help a household maintain housing. To qualify for this program the client needs to be behind on their rent and the landlord has to have served them a legal eviction notice. The client will also be required to have some type of income (employment, social security, TANF, unemployment, etc.) to qualify for assistance. Babysitting (or some other type of working) for cash will NOT be accepted as income. A client can only be assisted every two years through this program.

Transitional Housing is designed to move homeless families from streets and shelters into temporary housing and to maximize self-sufficiency, this program is for families only and they must be homeless in order to receive assistance. Homeless means living on the street, in an emergency shelter, or a place not meant for human habitation (cars, parks, sidewalks, abandoned building, etc.) Occupancy is limited.

Supportive Housing is designed to move homeless families/individuals with documented disabilities from streets and shelters into temporary housing and to maximize self-sufficiency. Enrollment is limited based on availability.

HOUSING OPTIONS | ABUNDANT FAITH CHRISTIAN CENTER

Offers supportive housing facilities for those in need. Each of these facilities offers supportive services, such as counseling, educational programs, quality childcare, and job training.


Haven House: Provides a home environment for women with or without children.
Haven Apartments: Permanent supportive housing that serves homeless women and their children who may be disabled.
Hope Place: Provides a home environment for families with low-to-moderate incomes.

TINY HOMES FOR VETERANS | CENTRAL ILLINOIS VETERANS COMMISSION

Provides qualifying veterans with affordable, permanent housing by building tiny homes. The veteran must be able to pay utilities, taxes, and insurance; there is no mortgage. At 10 years and 1 day, the veteran is given the title to the home.

Provides permanent supportive housing options for individuals who are homeless and have a documented mental illness or developmental disability.

BETHANY VILLAGE | BETHANY VILLAGE

Has 12 efficiency, 4 one-bedroom, and 3 two-bedroom units of transitional and permanent supportive housing. Provides food, shelter, clothing, supportive services, and assistance in finding employment. Prioritizes families with children. After two months of residency, applicants who demonstrate effective use of services may be approved for transitional housing up to one year.

Offers a Transitional Living Program for up to two years to assist homeless young women and their children to achieve stability in their lives by providing them with a stable home and intensive support services. Also offers aftercare services to help families as they transition into permanent housing.

Offers permanent affordable housing and supportive services for homeless families with dependent children in which the head of household or their child has a disability. The program enables families with severe and persistent issues, such as mental illness, chronic health problems, and substance abuse to achieve long-term housing stability.

The goal is to help individuals reach their fullest potential by encouraging and mentoring them.

Offers site-based permanent housing to adults living with a disabling condition (primarily HIV). Individuals living in permanent housing are afforded case management, advocacy, and other supportive services.

Also offers community-based permanent housing. Offers permanent housing to adults in apartments throughout the community. Provides housing to the chronically homelessness, HIV positive individuals, and disabled individuals.

A housing specialist works with families, who are at risk of losing custody of their children due to homelessness or near homelessness, to find safe, affordable housing, and keep the family unit intact. Supportive services are provided to help the family address the issues that brought them to this crisis and to help stabilize them before, during, and after housing placement.

The project partners believe that when homeless individuals with chronic medical conditions are able to focus their energies on healing and maintaining their health, rather than coping with the challenging circumstances that arise from living on the streets and in shelters, their health will improve. CHHP is for individuals who have frequent hospitalizations.
