HIV/Ryan White Case Management - AIDS Foundation of Chicago - Main Location

The AIDS Foundation of Chicago case management system provides a wide range of services, acting as a broker between clients and services. Their system identifies medical and social service needs and facilitates referrals to meet those needs; assists clients in applying for benefits, including housing, transportation, food and nutrition services; and facilitates access to emergency financial assistance funds in times of crisis. AFC has also invested significantly in a network of providers offering intensive case management and supportive housing options for people living with a wide range of chronic illnesses.

Corrections Case Management - Catholic Charities Archdiocese of Chicago - St. Sabina Parish

AIDS Foundation of Chicago's corrections case management program supports detainees who are HIV-positive as they are released from incarceration. The program connects clients from the moment they leave jail or prison. With support from AFC and other partners, those clients receive a primary care medical appointment, housing, substance abuse services, and/or mental health services.

HIV Resource HUB - Center on Halsted

The HUB provides assistance from professional staff with comprehensive information and direct connections to HIV services for people living with and vulnerable to HIV. Individual assessments will identify specific needs and connection to appropriate resources such as HIV/STD testing, PrEP support, primary care, housing assistance, medical transportation, emergency financial assistance, behavioral health and case management services and other essential services.

AIDS Foundation of Chicago's supportive housing programs are collaborations of community-based social service providers that simultaneously care for the medical and housing needs of people who are chronically ill (with HIV or another chronic disease) and homeless. The project partners believe that when homeless individuals with chronic medical conditions are able to focus their energies on healing and maintaining their health, rather than coping with the challenging circumstances that arise from living on the streets and in shelters, their health will improve. CHHP is for individuals who have frequent hospitalizations.

Community Supportive Living Systems Emerald House provides low-cost permanent housing with support services for those with disabilities. Clients who have difficulties with activities of daily living such as laundry, shopping, and cleaning are assigned personal assistants to help them complete these tasks. The fee is based on the resident's income.

Mental Health Counseling - South Side Help Center - Main Office

South Side Help Center (SSHC) provides mental health counseling for individuals and families with HIV or AIDS. The Mental Health Project provides counseling sessions that address major concerns, including disclosure to partners, family, and employer; what to expect as a result of a positive diagnosis; preparing for and dealing with the progression of the disease; and how/when to explain the ramifications of AIDS to their children. Support groups are also available for family and friends so that they may share experiences and help each other cope with the disease through moral support and camaraderie.

The Proactive Community Services (PCS) care coordination model utilizes an interdisciplinary team-based client approach. They combine primary care, medical case management, housing navigation/technician, ADAP (AIDS Drug Assistance Program) 340B prescription service with pharmacy assistance and medical adherence, and community health outreach with structured health education to promote HIV care continuum engagement and viral suppression.
