CCAC provides free case management to income qualifying pregnant women and children under the age of one. North Side Housing and Supportive Services provides on-site medical and mental health services to improve quality of life. It's comprised of HEAP and Clinical Mental Health services. These comprehensive services include, but are not limited to, one-on-one health education, care coordination, gift cards for grocery and one-on-one nutrition counseling, transit passes to medical appointments, and resources and referrals.

CCAC provides adults and children in North Lawndale, East and West Garfield Park and Near West Side with a healing space for community-based counseling.
