COMMUNITY CARE PROGRAM | SENIOR SERVICES OF WILL COUNTY

Helps seniors stay in their own homes by providing in-home and community based services. Aims to help seniors maintain their independence as an alternative to nursing home placement.

Services include adult Day Service, respite for family caregivers that include health, medication supervision, personal care, and recreational/therapeutic activities. Nutritious lunches and snacks are provided, including for special diets.

Automated Medication Dispenser Service (AMD) a portable mechanical system to dispense or remind about medications.

Emergency Home Response System (EHRS) 24 hour emergency communication link to outside assistance.

In-Home service assistance with household tasks, such as cleaning, planning and preparing meals, doing laundry, shopping, and running errands. Homemakers can also help with personal care, such as dressing, bathing, grooming, and following special diets.

Senior Companion offers scheduled visits by an older adult to provide friendship, companionship, and social activities.

ADULT DAY PROGRAM | INDEPENDENCE HEALTH AND THERAPY

Provides opportunities for socialization and therapeutic activities for individuals who may be isolated in the community or who require assistance due to physical, emotional, or mental health challenges. Individualized treatment plans are tailored for clients with physical, cognitive, social, or emotional needs.

Clients are offered door-to-door transportation and buses are staffed with drivers and aides certified in First Aid and CPR. Additionally, a light breakfast and full lunch are included in the day's program. Participants can attend one day or up to five days per week, depending on their schedules and needs.

This program serves individuals with:

-- Chronic health conditions.

-- Inability to manage independently.

-- Adults who are isolated or lonely.

-- Adults with a diagnosis of Alzheimer's or Dementia.

Provides older adults in need of additional supportive services, a place to participate in social and recreational activities while receiving necessary care and supervision. This program is meant for seniors who are in partial or total need of dependent help, socially isolated, physically or mentally impaired, and have an inability to manage personal care.

Provides a socially active program with activities that are designed intentionally to stimulate cognitive abilities in order to maintain and potentially improve memory. Also provides physical exercise to improve an individual's health and quality of life.
