Description

Breakthrough Urban Ministries' Behavioral Health program partners with individuals ages 5+ who are currently in Breakthrough programming to improve self-management skills and offer wellness services. The program provides on-site access to medical and mental health care, trauma-informed care, and resources for developing healthy behaviors.
