Description

Provides community-based support services for adults with severe mental health and co-occurring needs. Staff members meet the client at home or in the community to help them fulfill goals in their case plan. Provides services such as counseling, accompaniment, medication management, and job assistance. Services are delivered by a team of mental health professionals.
