Description

Pilsen Wellness Center's Project for Assistance in Transition from Homelessness (PATH) Program, provides community-based outreach and referrals for mental health, substance use, case management and other supportive services, to address the needs of adults ages 18-65+ and families, with serious mental illness and co-occurring substance use disorders, who are homeless or at imminent risk of becoming homeless. This program is funded by the Illinois Department of Human Services, Division of Mental Health.
