Application process

(1) Download and complete the forms available on the website, complete and mail them to [email protected] or fax to 773-442-0619 (2) After approval, applicant will get an account number to request an interpreter online. (3) For emergencies, submit online and call the 24-hour number at 866-251-0220.

Description

Provides audiology services, sign language interpreters, domestic violence counseling and for deaf, DeafBlind and hard of hearing (DDBHH) persons. Provides amplified phone demonstrations through ITAC's free amplified phone program. Audiological services include hearing testing and guidance in the selection and use of hearing aids. Also provides tax filing services through their VITA program for DDBHH persons.
