Eligibility

Open to individuals age 18 and older who have been diagnosed with an intellectual or developmental disability

Required documents

Proof of intellectual or developmental disability Proof of residence Photo ID Proof of registration on the PUNS list (this is done through Prairieland Service Coordination) Copy of Medicaid card and insurance card (if applicable) Community Choices may help obtaining required documents if clients do not have them

Description

Provides services to adults with intellectual and developmental disabilities. Helps manage DHS bureaucracy with families and connects Personal Support Workers (PSWs) with clients. Teaches 8-week classes on various life skills, including healthy relationships and sexuality, transportation, community and home safety, etc.

Self-Direction Assistance for People with the Home Based Support Waiver for People with I/DD:
