Eligibility

Must be currently insured and have coverage for medication seeking financial aid for; Must have a confirmed diagnosis and treatment plan; Must be living and receiving treatment in the United States; Household income must be at or below 300% or 400% of the Federal Poverty Guideline

Required documents

Proof of household income (tax returns, W-2 forms) Social Security Number DD-214 (for Veterans) Health Insurance information.

Provides direct financial assistance for co-payments, co-insurance and deductibles required by a patient's insurer for pharmaceutical treatments and/or prescription medications prescribed to treat and/or manage existing health conditions. Also offers scholarships for students who have survived cancer or chronic illness, as well as case management for patients with serious health conditions.
