Eligibility

Must be at least 18 years of age; Must be a current or prospective resident of Illinois, or a former resident of an Illinois long-term care facility; Friends and relatives of people who live in long-term care facilities; Staff and administrators of long-term care facilities with concerns about residents; Those who are considering long-term care facility placement in Illinois.
