(1) Call for information and for an assessment by phone (2) After-hours assessments can be completed at Ascension Alexian Brothers Behavioral Health Hospital in person or by phone. (3) Have your insurance information and treatment history ready (4) If accepted into the program, additional instruction will be provided (5) Bring required documents
