Required documents

Summary of Suspension (sometimes called Sworn Report) and Driving Abstract for Court Purposes are need to complete DUI Evaluation.

Description

Offers to perform evaluations and treatments that have been court ordered. Group programming is available for mental health, DUI, and substance abuse. Specific treatments for trauma and other difficulties are provided by staff who are specialists in their respective disciplines.
