Application process

1. Contact LifeLine's Mission Coordinators at 800-822-7972. Explain the patient's condition, the situation, and learn if LifeLine Pilots can assist the patient. 2. Complete the online application at lifelinepilots.org. This form can be completed by thepatient, a loved one, or a medical professional. See the list of information to prepare.3. If possible, assist in getting the physician's release signed and returned to LifeLine Pilots.4. LifeLine Pilots staff will go to work to find volunteer pilots. All communication will bebetween the Mission Coordinators and the designated person (patient, caregiver, or medicalprofessional). Once pilots have been confirmed, communication will be between the pilot(s)and the designated person.

LifeLine Pilots offers free air transportation for medical care to individuals facing financial or medical barriers to travel anywhere in the Midwest. Mission Coordinators are available to help determine patient eligibility. Flights can originate from any location in the Midwest and to destinations between 100-1,000 miles away. There is no limit to how many times a passenger can fly with LifeLine Pilots. Adult patients may take one companion to travel with them. Child patients (18 or under) may take two companions.
