Coordinates the following services: - Assessment for community-based supportive services - Access to in-home services - Access to adult day services - Access to congregate and home-delivered meals - Access to emergency home response and automated medication dispenser services - Housekeeping chores - Caregiver services - Benefits assistance - Nursing home pre-screening
