Eligibility
Individuals currently in need of an organ transplant (must be placed on the transplant waiting list, be in the process of evaluation for transplant, or must have received a transplant within the last 12 months); Must reside in Illinois or Lake County, Indiana (patients must have lived in those areas for at least 3 years consecutively); Must meet income guidelines; Must not be eligible for any other financial insurance subsidy or government insurance program; Must submit an annual re-certification form.
