Eligibility

Must live in DuPage County; Must be at least 19 years of age; Must not be eligible for any other type of health insurance; Household income must be at or below 250% of the federal poverty level (FPL).

Required documents

Photo ID (passport, state ID, driver's license or matricula consular) Proof of income (such as paystubs, W-2 forms, Social Security awards letter or pension) Proof of address (such as utility bill issued within last 60 days, driver's license or state ID issued within last 30 days, current lease or proof of current mortgage).
