Application process

This service is for parents with children age 5 and under. To apply, please download and fill out the application at the bottom of the website: https://grantawishincchicago.org/gaw-2022-baby-program. Once filled out, mail the application to P.O. Box 17698 Chicago, IL 60617-0698. Grant A Wish Inc. prefers a social worker or someone in a professional capacity to vouch for the particular needs of a mother or father, by sending in a letter on their agency letterhead advocating an interest in the Grant A Wish Baby Program and explaining the needs of their client. The agency must also provide proof of income unless the client has been given a waiver, to not send one.
