Description

Breakthrough Urban Ministries' Transitional Housing Program works alongside men and women experiencing homelessness to secure permanent or more stable housing and increase income within a 120-day goal through strong case management and connections to resources that address barriers including employment, substance abuse, mental and medical health needs, and legal aid. This housing-first, trauma-informed program provides a dedicated bed, care coordination, and housing retention support.
